Southwest Airlines Confirms Deadly Accident in NY to Dallas Flight

Tammie Jo Shults is being hailed a hero after landing a Southwest Airlines flight experiencing engine failure with more than 100 people on board

The pilot made an emergency landing; the woman later died from her injuries.

An initial investigation found evidence of metal fatigue where a fan blade had broken off. Investigators point to a hidden interior crack on the fan blade as the culprit.

Altogether, CFM 56-7B engines are on about 1,800 of the 737s in service in the US and about 6,400 worldwide.

USA aviation regulators are planning to require emergency inspections of one of the most popular jet engines in the world as a result of the fatal accident earlier this week on a Southwest Airlines plane.

Once the inspections are completed, CFM recommended to repeat the process every 3,000 cycles - about two years in airline service - but the FAA did not require such a measure.

Airlines using engines like the one that failed on a Southwest Airlines flight over Pennsylvania this week have 20 days to perform an inspection that could spot a flaw that likely led to the incident, federal authorities ordered Friday.

Some passengers that were on Tuesday's Southwest Flight 1380 received a $5,000 check from the airline, among other items.

A Colgan Air Flight, which was headed towards Buffalo, New York, crashed into a house.

At the same time, the pilots needed to communicate with air-traffic control to decide where to land and with flight attendants to gauge the damage to the plane.

"I know God was with her, and I know she was talking to God", Virginia Shults said.

When news broke of Shults piloting the crippled Southwest airliner, the name immediately jarred the mind of one Beeville resident.

Finke said she knew Shults was a standout the moment she met her. In both episodes, the first in 2016 and the one on Tuesday, fan blades in the planes' engines broke apart, sending shrapnel into the plane's body. A jet engine cycle comprises an engine start, takeoff and landing, and full shut down. CFM, which issued a warning about the possibility of flaws in the fan blades previous year, reported that it has manufactured 30,000 of the engines.

United Continental Holdings said that it had begun inspections earlier after a recommendation from CFM.

Inspections recommended by the end of August will affect an additional 2,500 engines.

Sumwalt told CNN that Kelly assured him Tuesday night that Southwest will be conducting "an aggressive ultrasonic inspection campaign for their entire fleet".

"This is a sad day and our hearts go out to the family and the loved ones of the deceased customer", he said. "That was a very hard position to be in, to think who is most important to your life and in what order?"

But statements about Mrs. Riordan that were closer to home for the parishioner of Our Lady of the Annunciation Catholic Church in Albuquerque and mother of two children at Annunciation School were issued by her family, who called her their "bedrock", and her children's school, which described Mrs. Riordan as an "integral member of our school community".

"The FAA's top priority is, and always will be, ensuring operators follow our rules and regulations", the agency said in a statement.
